The four ways enterprise AI engagements fail.

Almost every failed AI engagement we've seen post-mortem'd into one of four buckets. Spotting them early is the difference between deployed and shelved.

  • Wrong problem: working on a workflow nobody actually cares about.
  • Wrong data: building on top of data that's missing, dirty, or inaccessible.
  • Wrong adoption: nobody uses the thing because nobody owns the rollout.
  • Wrong governance: legal, compliance, or security blocks production at the eleventh hour.

Wrong problem.

The team picks a workflow that looks technically interesting and ships AI for it — and it turns out nobody on the operations side wakes up worrying about that workflow. Beautiful demos. No production usage. The fix: anchor every engagement on a workflow whose owner can articulate the cost of doing it badly today.

If the operating leader can't quantify the pain, the AI won't move the needle. Pick a different problem.

Warning signs: the project champion is in IT or innovation, not the line of business. The operating leader hasn't been in the room. The success metric is a model accuracy number, not a workflow outcome.

Wrong data.

The data the system needs is fragmented across systems no one wants to integrate, gated by a security review that takes a quarter, or just doesn't exist in the form the AI requires. By the time the data is actually accessible, the deadline has passed, the budget is gone, and the project is dead.

Make data access a phase-zero deliverable. If you can't get the data in week two, the project is at risk.

Warning signs: nobody can name the source-of-truth system for the workflow. The data sample shown in the kickoff is a CSV someone exported by hand. Data ownership crosses three VPs and none of them are at the table.

Wrong adoption.

The system ships. It works. Nobody uses it. Because no one owns the rollout, no one trains the users, no one integrates it into the daily workflow, and no one cares if it's used or not. The application becomes a forgotten URL.

Adoption is its own track of work, with its own owner, starting before the application ships. If there's no rollout plan with a named owner, the project will fail at the finish line.

Warning signs: the rollout plan is "send a launch email." There's no named owner for adoption. The system isn't integrated into the existing screens the team already uses. End-users haven't seen the application before launch day.

Wrong governance.

The application is built. Production day arrives. Legal hasn't reviewed the data flow. Compliance hasn't approved the model provider. Security is still finding the audit logs. The application sits in staging for six months while reviews queue.

Bring legal, compliance, and security in at week zero, not week twelve. Their constraints shape the architecture. Better to know early than to rebuild later.

Warning signs: security wasn't invited to the kickoff. There's no signed BAA / DPA before data starts flowing. Audit logs are an afterthought. The model-provider data agreement is "we'll figure that out later."
